(If your church does not have a My Healthy Church account number, please call 1-855-642-2011 between the hours 8:00 am and 4:30 pm (CST) Monday-Friday).
If your church does not have an account number, please call 800-641-4310 between the hours of 7:00 AM and 5:30 PM (CST) Monday-Friday.
2. Number of boys in each age group
(Must have at least two leaders and three boys TOTAL)
